A Tapestry of Global Health and Heritage: The Lumiere and Harvard’s Exploration of the Dominican Republic


In the vibrant heart of the Caribbean, the Dominican Republic stands as a beacon of rich culture and deep historical roots, all while facing the modern-day triumphs and challenges of global health. Recently, Lumiere had the honor of collaborating with Postgraduate Public Service Fellows and Mindich Fellows both from Harvard University on a journey through this beautiful, lively country. This wasn't just a trip; it was a deep dive into the dynamics of global health within the complex socio-economic and cultural landscape of the Dominican Republic.


Venturing Through Time and Tradition

Our adventure began in the bustling capital of Santo Domingo. This city, alive with culture and history that stretches back over five centuries, offered us a firsthand look at the Dominican Republic's health system against a backdrop of colonial heritage. Strolling through its historic streets, we quickly grasped the importance of the cultural, political, and economic contexts that have sculpted this nation and its impact on public health and population health.

As we moved into the lush Cibao region, the narrative shifted. Here, the striking differences in healthcare accessibility became starkly apparent. In remote rural towns, amidst breathtaking landscapes, we saw the real face of healthcare delivery challenges. The serene beauty of the countryside stood in sharp contrast to the pressing healthcare needs of its residents.

Diving deeper into these communities, in a small town called Manacla, we encountered rural clinics operating on the fringes of the healthcare system. Many of these clinics serve primarily as pharmacies, with a limited selection of medicines and no clinical staff to provide care. This reality highlighted a significant gap in the healthcare system, where access to comprehensive medical services is not just limited but often non-existent, impacting not only immediate health concerns but also the broader spectrum of public health and disease prevention.

Our journey through the Cibao region also allowed us to engage in valuable primary data collection efforts. By directly interacting with these communities and observing the operations of rural clinics, we collected critical primary data on public health initiatives, vaccination rates, and the incidence and prevalence of communicable diseases, which are crucial for understanding the effectiveness of public health measures in these areas and to develop innovative initiatives to address challenges.

The absence of clinical staff and reliance on limited pharmacy services underscored the dire need for enhanced healthcare infrastructure and professional medical support. This situation is a clarion call to action, highlighting the urgency of bridging the healthcare provision gap to ensure no community is left behind.

Bridging Borders and Building Understanding

Our mission led us to Dajabón, a town at the border between the Dominican Republic and Haiti. There, the bustling Bi-National Market embodies the vibrant trade and complex relations between the Dominican Republic and Haiti. Before recent governmental restrictions, Haitians were allowed into a predetermined perimeter on the Dominican side, a practice tightly controlled by military forces. This market, welcoming thousands of Haitians daily, serves as a critical economic lifeline, allowing them to buy and sell goods, thus providing a much-needed source of income. Between 2,000 to 5,000 Haitians step into the market in the Dominican Republic, weaving through the hours from 8:00 AM to 4:00 PM in a dance of commerce and livelihood.

However, the market's crowded, chaotic nature, exacerbated by hot, humid conditions and limited sanitation, poses significant public health risks. The potential for the spread of communicable diseases in such settings highlights the urgent need for collaborative public health solutions that transcend borders.

Witnessing the dynamic interaction at the Bi-National market deepened our understanding of the interconnectedness of health and economy, particularly at the international level. It emphasized the critical need for innovative approaches to manage public health risks in border regions, where economic vitality and health concerns are deeply intertwined.

Moreover, our engagement in the Dominican Republic extended to meaningful dialogues with leaders of various organizations and government institutions throughout the Dominican Republic. These interactions nurtured relationships and encouraged avenues for future business and diplomatic collaborations and policy recommendations, underscoring the importance of partnership in addressing global health challenges.


Reflecting on a Rich Tapestry of Experiences

Our exploration through the Dominican Republic was more than a field research endeavor; it was a journey through the heart of global health, enriched by the country's cultural and historical heritage. Beyond the data and observations, it was the personal stories and the strong sense of community that profoundly impacted us, reminding us that global health is fundamentally about people and their stories.

Forging Ahead with Renewed Purpose

As we reflect on our experiences, we are reminded of the intricate tapestry of global health challenges and the importance of collaborative efforts to address them. This journey has not only enriched our understanding but has also strengthened our commitment to making a meaningful impact in global health.
